BMJ publiceerde recent een overzichtsartikel over de meerwaarde van til technieken. Wat blijkt goed leren tillen heeft geen invloed op het ontstaan van rugpijn, dit integenstelling tot wat de ARBO verteld. Hieronder de samenvatting van het artikel.

Objectives To determine whether advice and training on working techniques and lifting equipment prevent back pain in jobs that involve heavy lifting.

Data sources Medline, Embase, CENTRAL, Cochrane Back Group’s specialised register, CINAHL, Nioshtic, CISdoc, Science Citation Index, and PsychLIT were searched up to September-November 2005.

Review methods The primary search focused on randomised controlled trials and the secondary search on cohort studies with a concurrent control group. Interventions aimed to modify techniques for lifting and handling heavy objects or patients and including measurements for back pain, consequent disability, or sick leave as the main outcome were considered for the review. Two authors independently assessed eligibility of the studies and methodological quality of those included. For data synthesis, we summarised the results of studies comparing similar interventions. We used odds ratios and effect sizes to combine the results in a meta-analysis. Finally, we compared the conclusions of the primary and secondary analyses.

Results Six randomised trials and five cohort studies met the inclusion criteria. Two randomised trials and all cohort studies were labelled as high quality. Eight studies looked at lifting and moving patients, and three studies were conducted among baggage handlers or postal workers. Those in control groups received no intervention or minimal training, physical exercise, or use of back belts. None of the comparisons in randomised trials (17 720 participants) yielded significant differences. In the secondary analysis, none of the cohort studies (772 participants) had significant results, which supports the results of the randomised trials.

Conclusions There is no evidence to support use of advice or training in working techniques with or without lifting equipment for preventing back pain or consequent disability. The findings challenge current widespread practice of advising workers on correct lifting technique.

Het was vandaag weer eens zover. Een patiënt ligt bij mij op de SEH en ik krijg het weer niet voor elkaar om de huisarts van deze patiënt te bereiken… gelukkig is er dan altijd nog de apotheek die uitkomst kan brengen.. gelukkig ik heb de medicatie nu alleen nog alle operaties en andere voorgeschiedenis. Bel morgen de huisarts wel weer, en dan was het nu woensdag … probeer dit maar op een vrijdag middag…..

Slecht bereikbare huisarts kan boete krijgen
DEN HAAG – Huisartsen die in spoedgevallen slecht bereikbaar zijn, kunnen de Inspectie voor de Gezondheidszorg (IGZ) op hun dak krijgen. Die kan hen in het uiterste geval een boete opleggen.
Dat zei minister Ab Klink (Volksgezondheid) woensdag in de Tweede Kamer in een reactie op een voorstel van PVV-Kamerlid Fleur Agema om huisartsen die bij spoedgevallen niet thuisgeven hard aan te pakken.
De minister onderschreef Agema’s stelling dat het niet door de beugel kan dat huisartsen in 4 procent van de spoedgevallen niet te bereiken zijn. Maar haar idee om al na drie verzuimen een schorsing op te leggen, ging de minister veel te ver.

Een zeer opmerkelijk bericht gevonden op gezondheidsnet.nl. Het blijkt zo te zijn dat mannen steeds meer borstverkleiningen laten doen. De vraag die ik er dan automatisch bij heb is;”Zijn er dan zoveel mannen met een cup-maatje te veel?”

Mannen stappen steeds vaker naar de plastisch chirurg om hun borsten te laten verkleinen. Het lijkt erop dat mannen zich meer en meer bewust worden van hun uiterlijk.
Ook andere ingrepen zijn in trek bij de mannen, met op nummer één de neuscorrectie. Liposuctie is mateloos populair, om de bierbuik en zwembandjes te verwijderen. Maar de borstverkleining groeit het hardst, met acht keer zoveel operaties vergeleken met drie jaar geleden.
Bij de vrouw is de borstverkleining overigens ook terrein aan het winnen. Al is de borstvergroting nog steeds de meest populaire operatie. Dokters zien ook een toename in andere ingrepen. De drempel om plastische chirurgie te nemen lijkt af te nemen.
Tegenwoordig zijn er vele opties die minder ingrijpend zijn – zoals botox. Hierdoor komen mensen sneller de kliniek binnen en wordt de volgende stap (chirurgie) ook makkelijker gezet. Daarbij blijken mannen zich dus ook steeds meer met hun uiterlijk bezig te houden.

The American Contact Dermatitis Society has proclaimed nickel as its 2008 Allergen of the Year because of its rise as a cause of significant contact dermatitis in the United States, particularly among children.

“With the rising incidence of nickel allergy in the United States and elsewhere, high nickel sensitization rates documented in children, and a resurgent issue of biomedical device complications (specifically, nickel allergy) due to metal, to dismiss nickel’s importance and relevance to public health and skin disease would be a mistake,” Dr. Kathryn A. Zug, wrote in an article that is scheduled to be published in the January/February issue of Dermatitis.

This year’s winner can be found in jewelry, coins, buckles, pant snaps, tools, and other products used in everyday life, noted Dr. Zug of the of the department of dermatology at Dartmouth Hitchcock Medical Center, Lebanon, N.H.

Data from the North American Contact Dermatitis Group (NACDG) have revealed that the number of patients who patch tested positive for nickel grew from 10.5% in 1985-1990 to 18.8% in 2003-2004. And of 391 children patch tested by the NACDG from 2001-2004, 28% had a positive patch test to nickel and 26% were thought to have a nickel allergy of either current or past relevance.Bron:ACEP
