A fundamental tension exists between safety in the common case and security under adversarial conditions for wireless implantable medical devices. We propose a class of new, fail-open defensive techniques for implantable medical devices that attempt to strike a balance between these two goals. We refer to these defensive techniques as Communication Cloakers. Cloakers are externally worn devices, much like computational Medical Alert bracelets. Cloakers protect the security of an IMD when worn, but allow for open access during emergencies if removed.

Belgische Kamerleden vrezen dat veel adresloze Belgen misbruik maken van sociale voorzieningen. Onder de Belgen zonder adres zijn personen die al heel erg lang spoorloos zijn. Er staan 2535 personen op die ouder zijn dan 110 jaar. De tien oudste adreslozen zijn op papier althans de leeftijd van 140 jaar reeds gepasseerd.

Verhuizing Deventerziekenhuis


In de week van 20 augustus 2008 is het zover! Dan start de verhuizing van het Deventer Ziekenhuis en gaan we over naar een schitterend nieuw gebouw.


Vanaf 20-08-08 wordt dan de feitelijke verhuizing in ongeveer twee weken tijd afgerond. De patiëntenverhuizing vindt plaats op één dag. Dat zal waarschijnlijk eind augustus zijn. De exacte datum moet nog worden bepaald. Circa honderd patiënten van het Deventer Ziekenhuis worden volgende week met vrachtwagens naar de nieuwe locatie van de instelling verhuisd. De instelling heeft elders in de stad een nieuwe locatie gebouwd.

Acht vrachtwagens die tijdelijk zijn omgebouwd tot rijdende verpleegkamers rijden de patiënten ’s nachts en ’s ochtends naar de nieuwe locatie. De politie begeleidt het transport om ongehinderde doorgang zoveel mogelijk te garanderen, meldde de gemeente Deventer maandag.

De route blijft tijdens de bijzondere verhuizing open voor het verkeer, aldus de gemeente.

Redbull veroorzaakt hartproblemen

Red Bull may claim to “give you wings” but drinking too much of the popular energy drink may also lead to heart damage, a study suggests.

A study of 30 university students aged between 20 and 24 years old found that drinking just one 250ml sugar-free can of the caffeinated energy drink increased the “stickiness” of the blood and raised the risk of blood clots forming.

Using tests to measure blood pressure and the state of blood vessels around the body, the Australian researchers said that after drinking one can participants had shown a cardiovascular profile similar to that of someone with heart disease.

Red Bull today emphatically denied that the drink, which is distributed to 143 countries worldwide, was dangerous. In a statement, it said that Red Bull had been proved safe by “numerous scientific studies”, and that it had never been banned from anywhere it had been introduced.

“After one can it seemed to turn the young individual into one with more of the type of profile you would expect to see with someone with cardiovascular disease,” he said. “People who already have existing cardiovascular disease may want to talk to their physician before they drink Red Bull in future.”

Previous studies have examined the effects of consuming Red Bull, one can of which contains 80mg of caffeine — around the same as a cup of filter coffee — and taurine, an amino acid commonly used in energy drinks.

An estimated 330 million litres of energy drinks were consumed in Britain last year, a market worth £1 billion. However Red Bull is banned in countries such as Norway, Uruguay and Denmark because of health fears.

Last year a research team from Henry Ford Hospital in Detroit presented a paper to the American Heart Association suggesting that energy drinks may boost heart rates and blood pressure levels. The results, from a small study, prompted them to warn against consumption by those with cardiovascular problems.

Earlier this year a teenager from Darlington was sent to hospital after drinking eight cans of Red Bull. Paramedics reported that the 15-year-old suffered heart palpitations.

Last year a large study of American college students found that those who drank energy drinks mixed with alcohol during a night out were twice as likely to be injured or otherwise come to harm than those who consumed alcoholic drinks on their own.

Dr Willoughby said that he was prompted to conduct the study after recent reports of deaths in young people who had consumed large amounts of the energy drink before they died.

The high levels of caffeine in Red Bull was suspected to be a contributing factor in the death of a 40-year-old man in Oxford who suffered a fatal heart attack in April after collapsing at an Asda store where he worked. In 2007 an 18-year-old British basketballer died after consuming three cans of the drink, and in July 2001 and 33-year-old Australian man died of a heart attack after drinking a pitcher of Red Bull and vodka.

“There was enough anecdotal evidence to catch my attention,” Dr Willoughby said. He added that previous studies had shown caffeine and taurine to have an effect on blood platelets and heart function, sometimes with beneficial results, such as boosting endurance while exercising.

But, he added, “maybe there’s something quirky about the effect of the combination of the two in the drink which is causing this reaction, this is what we need to look at next.”

The results shocked the 30 students, some of whom drank up to eight cans a night to help them stay awake to study, and many now refuse to consume the energy drink again. Dr Willoughby said he now aims to extend this preliminary study to a larger group of students to verify the results.

Linda Rychter, a spokeswoman for Red Bull in Australia, said that the report would be assessed by the company’s head office in Austria. “The study does not show effects which would go beyond that of drinking a cup of coffee. Therefore, the reported results were to be expected and lie within the normal physiological range,” she added.

Red Bull, which has the catchphrase “Red Bull gives you wings”, was created in Austria in the 1980s. The company last year sold 3.5 billion cans of the drink in 143 countries. Cans of Red Bull already carry health warnings advising customers not to drink more two cans a day.

Enterovirus infect mogelijk medeveroorzaker ADHD

Children who had had an enterovirus 71 infection involving the central nervous system were significantly more likely to have symptoms of attention-deficit/hyperactivity disorder than were matched controls in a prospective study that evaluated children at 3-7 years after the infection.

Although herpes simplex encephalitis and other CNS infections can affect neurodevelopment and cognitive function, the authors said that this is the first study to follow up long-term behavioral outcomes or ADHD-related symptoms in children after an enterovirus 71 (EV71) CNS infection.

The findings have “clearly demonstrated the association between the EV71 CNS infection and increased symptoms of inattention, hyperactivity, oppositional defiance, internalizing problems, and increased likelihood of ADHD diagnosis,” concluded Dr. Susan Shur-Fen Gau of National Taiwan University, Taipei, and her associates.

The results also support their hypothesis that children who have had an EV71 CNS infection “are more likely to have ADHD-related symptoms regardless of IQ,” (Pediatrics 2008;122:e452-8).

The study used standardized mother- and teacher-rated scales to evaluate ADHD symptoms and other emotional and behavioral problems in 86 children (51 boys and 35 girls; aged 4-16 years) who had had an EV71 CNS infection at the mean age of 2.5 years, and in 172 controls in the same neighborhoods or schools who were matched for gender, age, school performance, and parental education levels. Among the children with the infections, CNS involvement had been mild in 42 cases (aseptic meningitis) and was severe in 35 cases (encephalitis, poliomyelitislike syndrome, or encephalomyelitis); the other 9 children had cardiopulmonary failure after CNS involvement. The children had been diagnosed with the infections from 1998 to 2003 at Chang Gung Children’s Hospital, Taoyuan, Taiwan, and National Taiwan University Hospital; there was an epidemic of EV71 infection in Taiwan in 1998.

Scores on teacher- and mother-rated scales of inattention, hyperactivity-impulsivity, oppositional symptoms, and ADHD index were significantly higher among the children with the EV71 infection, compared with controls. In the former group, 20% had elevated ADHD-related symptoms, compared with 3% of controls, a significant difference.

The maternal reports provided some evidence that children with the EV71 CNS infections had more internalizing problems, but this needs to be studied further, the investigators said. There was no correlation between the age at which the child had the EV71 infection, any laboratory data, or the severity of CNS involvement with the severity of ADHD-related symptoms, a finding that the investigators said was surprising. They speculated that the EV71 infection may involve the prefrontal-striatum-subcortical area of the brain, or another area that is related to the core symptoms of ADHD.

The investigators acknowledged some limitations of the study, including the inability to assess ADHD symptoms in the children before the CNS infection, and said that more studies were needed to confirm whether the increase in ADHD symptoms was specific to EV71 or also occurred with other microorganisms.

Nevertheless, they concluded that an EV71 CNS infection “may affect long-term regulation of attention and emotion and cause hyperactivity-impulsivity in children,” and recommended that children with these infections be assessed early to identify ADHD symptoms and emotional and behavioral problems, because “early intervention may prove beneficial for their future performance.”

Innovatie medische sector blijft achter

De innovatie in de medische sector blijft in Nederland achter. Oorzaak is de kloof tussen de ontwikkeling van kennis en toepassing ervan.
Dit concluderen onderzoekers van de Rotterdam School of Management. Zij stellen daarom dat het rendement uit investeringen in onderzoek en ontwikkeling van medische producten fors omhoog moet. Hiervoor is meer samenwerking nodig, effectief leiderschap en een professioneel middenkader.
De studie ‘Meer rendement uit R&D’ is uitgevoerd in opdracht van het ministerie van VWS.

VVD: zorg moet commerciëler in de komende 15 jaar

De VVD wil dat er snel maatregelen worden genomen om de zorg in Nederland bereikbaar te houden.
Dit heeft VVD-vicefractievoorzitter Edith Schippers maandag laten weten bij de presentatie van de VVD zorgnota in het Zuwe Hofstede Ziekenhuis in Woerden. In deze nota geeft de VVD Tweede Kamerfractie haar visie op zorg in Nederland voor de komende 15 jaar.
De VVD laat in de nota weten dat de bereikbaarheid en kleinschalige persoonsgerichte zorg in Nederland onder druk staan. Er moeten snel maatregelen genomen worden om deze te behouden. Zo wil de VVD dat de indicatiestelling voor de AWBZ komt te vervallen. Schippers:”De indicatiestelling is een nodeloos ingewikkeld en ondoorzichtig systeem. De VVD wil dat de arts bepaalt wie er in aanmerking voor de AWBZ komt”. De VVD wil ook dat het Persoongebonden Budget blijft bestaan. Dit Persoonsgebonden Budget staat momenteel ter discussie. Schippers:”Mensen moeten zelf kunnen kiezen hoe ze hun zorg willen regelen. Als zij niet tevreden zijn met een bepaalde begeleider, dan moeten ze over kunnen stappen. Instellingen zullen harder hun best moeten doen om de mensen aan zich te binden, wat maakt dat de kwaliteit van zorg verbetert”.
De zorgnota schetst een toekomstbeeld over hoe de zorg in 2020 in Nederland moet functioneren. Dit globale beeld moet richting geven aan de besluiten die vandaag worden genomen. Het bieden van perspectief aan de sector is cruciaal omdat dagelijks beslissingen worden genomen over investeringen, welke mogelijkheden worden benut, et cetera. Onzekerheid is dood in de pot. De overheid geeft momenteel te weinig zekerheid. Helderheid is snel geboden. De VVD-fractie geeft met deze nota helderheid over het toekomstperspectief dat zij wil geven aan de sector, de mensen die erin werken en ervan afhankelijk zijn. (Medicalfacts)
