In het Universitair Medisch Centrum Groningen is onlangs een bijzondere vorm van multi-orgaantransplantatie uitgevoerd. Een jongetje van vier ontving tegelijkertijd een nieuwe lever, alvleesklier, dunne darm en dikke darm. Het is voor het eerst in Nederland dat een kind een darmtransplantatie onderging. Ook een dergelijke complexe procedure waarbij meerdere buikorganen getransplanteerd worden, is uniek in ons land. Chirurgen en anesthesiologen hadden, samen met ondersteunende teams, ruim twaalf uur nodig om de gehele procedure uit te voeren. 

De getransplanteerde jongen lijdt aan een zeer zeldzame aangeboren darmziekte (microvillous inclusion disease), waardoor de darm geen voeding kan opnemen. Jaarlijks worden enkele kinderen geboren met deze aandoening. Kinderen met deze ziekte zijn afhankelijk van kunstmatige voeding die ze door een infuus krijgen (totale parenterale voeding, TPV). Doorgaans bereiken zij de volwassen leeftijd niet. Dankzij de begeleiding van het TPV team van het Academisch Medisch Centrum in Amsterdam heeft het jongetje de afgelopen jaren een relatieve normale groei en ontwikkeling doorgemaakt.

Echter, door infecties en een bijkomende leverbeschadiging kon niet doorgegaan worden met de TPV. De oplossing werd gevonden in deze unieke transplantatie, waarbij de jongen naast een dunne- en dikke darm- ook een levertransplantatie onderging. De lever en de darmen zijn samen met de tussenliggende alvleesklier getransplanteerd nadat de eigen zieke lever en darmen waren verwijderd. In Nederland was nog niet eerder een dikke darm met een dunne darm meegetransplanteerd.

Kinderartsen, chirurgen, maag-, darm- en leverartsen, anesthesiologen, medisch microbiologen en pathologen van het UMCG hebben nauw samengewerkt om deze bijzondere multi-orgaantransplantatie uit te kunnen voeren. Daardoor zijn de transplantatieprocedure en de postoperatieve zorg goed verlopen. Op dit moment gaat het goed met de jongen. Zijn nieuwe darmen functioneren goed en nemen voeding op, die hij per sonde krijgt. Het jongetje zal op korte termijn dus voor het eerst gewoon voedsel kunnen eten.

Het UMCG is in Nederland het enige centrum dat darmtransplantaties uitvoert. Naast het AMC is ook het UMC St Radboud gespecialiseerd in totale parenterale voeding. Deze drie UMC’s werken nauw samen om patiënten met deze en andere darmafwijkingen de beste, vaak zeer gespecialiseerde zorg te kunnen bieden. 

UMCG Transplantatiecentrum
Het UMCG is een van de weinige ziekenhuizen in de wereld en het enige ziekenhuis in Nederland, dat alle vormen van orgaantransplantaties verricht. Niet alleen hart-, nier-, long-, lever-, pancreas- en darmtransplantaties, maar ook gecombineerde transplantaties zoals hart/longtransplantaties, long/levertransplantaties en nier/pancreastransplantaties worden door het UMCG uitgevoerd. Patiënten kunnen ook in het UMCG terecht voor transplantaties van huid, hartkleppen, hoornvlies en bot. Patiënten bij wie het beenmerg onvoldoende bloedcellen aanmaakt of die een kwaadaardige bloedziekte hebben als leukemie of lymfklierkanker, biedt het ziekenhuis stamceltransplantaties. Uiteraard is transplantatie alleen mogelijk als er mensen bereid zijn zich te laten registreren als orgaandonor. (Laat dit bericht voorlezen)

Children whose mothers reported smoking during pregnancy were 2.4-fold more likely to develop ADHD than were those whose mothers reported not smoking during pregnancy. Compared with children in the first tertile of blood lead level, those in the second tertile were 1.7-fold more likely to develop ADHD. That increased to 2.3-fold for children in the third tertile. All those results were statistically significant.

In terms of absolute risk, 7% of the children whose mothers reported not smoking during pregnancy met criteria for ADHD, compared with 17% of those whose mothers did smoke during pregnancy. Of those in the first tertile of blood lead levels, 5% met criteria for ADHD, compared with 9% in the second tertile and 14% in the third tertile.

In the preliminary round, the selective adenosine A1 receptor blocker rapidly improved acute heart failure symptoms, prevented further worsening of renal function, and reduced the combined rate of death or hospital readmission within 60 days, Dr. Barry M. Massie reported at the annual meeting of the American College of Cardiology.

“This is the first evidence that an intervention to prevent renal impairment may positively affect acute symptoms and intermediate-term outcomes in patients hospitalized with acute heart failure. These pilot data have important therapeutic implications. Better treatment of acute heart failure patients with progressive renal dysfunction requiring diuresis is a significant unmet medical need,” observed Dr. Massie, professor of medicine at the University of California, San Francisco, and chief of cardiology at the San Francisco VA Medical Center.

Heart failure is the No. 1 cause of hospitalization in patients aged older than 65 years. Guidelines call for the routine use of diuretics in these acutely decompensated patients. But diuretics can be associated with worsening renal function, especially in patients with underlying chronic kidney disease. And worsening renal function is itself associated with a worse prognosis.

Adenosine stimulates sodium reabsorption in the proximal tubule. In the setting of increased sodium load presented to the distal tubule during diuresis, adenosine also mediates reduced renal blood flow and renal function. Rolofylline has the opposite effects.

In the pilot dose-finding study, 301 patients were randomized to intravenous rolofylline at 10, 20, or 30 mg or to placebo infused daily with loop diuretics over a 4-hour period on the first 3 days of hospitalization.

The primary study end point was therapeutic success defined as patient-reported moderate or marked improvement in dyspnea on day 2 or 3. The dose-related success rate ranged from 37% with placebo to 53% with rolofylline at 30 mg/day. The failure rate (defined as worsening heart failure symptoms or renal function) ranged from 38% on placebo to 16% with high-dose rolofylline.

The 60-day combined end point of death or rehospitalization for renal or cardiovascular causes was 33% with placebo, 32% with rolofylline at 10 mg, 24% at 20 mg, and 19% at 30 mg.

Rolofylline’s seizure potential was a potential concern because the adenosine A1 receptor downregulates electrical activity in the CNS. Patients at high seizure risk because of past history were excluded from the trial, and those deemed at intermediate risk received daily prophylactic lorazepam. No seizures occurred in the study. Indeed, rolofylline’s side effect profile was essentially the same as placebo, according to Dr. Massie.

The phase III trial, called PROTECT, is ongoing. To date, roughly 900 of a planned 2,000 patients have been enrolled. Only the 30-mg dose of rolofylline is being used, and trial end points are the same as in the pilot study. Results should be available early next year.

CHICAGO (EGMN) - Any detectable plasma cardiac troponin T level in ambulatory patients with stable coronary artery disease is an independent predictor of future cardiovascular events, according to the Heart and Soul Study.

“Our data suggest that [troponin T] testing may have potential as a risk-stratifying tool in the management of stable CAD,” said Dr. Bill Pei-Chin Hsieh reported at the annual meeting of the American College of Cardiology. “It gives added predictive value beyond the traditional risk factors for predicting all the adverse outcomes you could think of. A positive troponin T [TnT] could be a reason to pursue a more aggressive management strategy.”

An elevated cardiac TnT - defined as any measurable level using the commercially available third-generation Roche immunoassay - is not uncommon in the setting of stable CAD. Levels of 0.01–0.72 mcg/mL were identified in 6.2% of 988 patients enrolled in the Heart and Soul Study, a National Institutes of Health–sponsored study whose primary aim was to investigate the role of psychosocial factors in CAD. Participants had to have a history of MI or coronary revascularization, angiographic evidence of greater than 50% stenosis in at least one coronary artery, or demonstration of exercise-induced ischemia on treadmill or nuclear testing.

Background  Particulate air pollution has been linked to heart disease and stroke, possibly resulting from enhanced coagulation and arterial thrombosis. Whether particulate air pollution exposure is related to venous thrombosis is unknown.

Methods  We examined the association of exposure to particulate matter of less than 10 µm in aerodynamic diameter (PM₁₀) with deep vein thrombosis (DVT) risk in 870 patients and 1210 controls from the Lombardy region in Italy, who were examined between 1995 and 2005. We estimated exposure to PM₁₀ in the year before DVT diagnosis (cases) or examination (controls) through area-specific mean levels obtained from ambient monitors.

Results  Higher mean PM₁₀ level in the year before the examination was associated with shortened prothrombin time (PT) in DVT cases (standardized regression coefficient [β] = –0.12; 95% confidence interval [CI], –0.23 to 0.00) (P = .04) and controls (β = –0.06; 95% CI, –0.11to 0.00) (P = .04). Each increase of 10 µg/m³ in PM₁₀ was associated with a 70% increase in DVT risk (odds ratio [OR], 1.70; 95% CI, 1.30 to 2.23) (P < .001) in models adjusting for clinical and environmental covariates. The exposure-response relationship was approximately linearover the observed PM₁₀ range. The association between PM₁₀ level and DVT risk was weaker in women (OR, 1.40; 95% CI, 1.02 to 1.92) (P = .02 for the interaction between PM₁₀ and sex), particularly in those using oral contraceptives or hormone therapy (OR, 0.97; 95% CI, 0.58 to 1.61) (P = .048for the interaction between PM₁₀ level and hormone use).

Conclusions  Long-term exposure to particulate air pollution is associated with altered coagulation function and DVT risk. Other risk factors for DVT may modulate the effect of particulate air pollution.

De test, die sinds september vorig jaar verkrijgbaar is in ons land, (België, red.) meet de hoeveelheid PCA3 in de urine van een man. PCA3 is een specifiek prostaatgen dat alleen in grote mate voorkomt wanneer iemand prostaatkanker heeft. Daardoor wordt de test als meer betrouwbaar beschouwd dan andere methodes. Nu blijkt dat de test ook kan nagaan in welk stadium de prostaatkanker zich bevindt en om welke graad van kanker het gaat.

“De test kan bijdragen tot het differentiëren tussen patiënten die in aanmerking komen voor een actieve monitoring en patiënten die een agressieve behandeling nodig hebben”, zegt professor Van Poppel, uroloog bij het UZ Gasthuisberg. De test kan met name een waardevol instrument zijn om te verzekeren dat agressieve behandelingen of invasieve biopsieën beperkt kunnen blijven tot de patiënten die dit het meeste nodig hebben.
